Cognihealth provides non-medical home care, companion care, personal care, dementia support, respite care, and lifestyle assistance.
Care plans are customized based on client needs.
Services do not include medical diagnosis or treatment.
For emergencies, call 911.
2. Eligibility
Users must be 18 years or older or a legal representative of the client.
Information provided must be accurate and truthful.
3. Client Responsibilities
Clients agree to:
Provide accurate care information.
Maintain a safe environment.
Notify CogniHealth of any changes in condition or care needs.
Treat staff respectfully.
CogniHealth may discontinue services for:
Safety concerns
Harassment or abusive behavior
Illegal activity
Non-payment
4. Scheduling & Cancellations
Scheduling is managed through our Care Management Team.
Advance notice for cancellations is required as outlined in the service agreement.
Repeated late cancellations may incur fees.
5. Payment & Billing
Fees are outlined in the service agreement.
Payment must be made as agreed.
Non-payment may result in service termination.
Insurance reimbursement is not guaranteed unless pre-authorized.
6. SMS/Text Messaging
By opting in, users may receive service-related texts, including:
Schedule updates
Caregiver arrival alerts
Visit follow-ups
Message and data rates may apply.
Reply STOP to opt out or HELP for assistance.
CogniHealth does not send promotional or marketing text messages.
7. Privacy & Data Protection
All personal information is handled according to our Privacy Policy.
CogniHealth does not sell, rent, or share personal data with third parties for marketing purposes.
8. Social Media Use
Cognihealth may share updates on official social media pages. Engagement is voluntary.
Privacy Policy links may be pinned for visibility.
No personal data is collected from social media for marketing.
9. Intellectual Property
All website content, branding, and materials are the property of CogniHealth, LLC.
Unauthorized reproduction or distribution is prohibited.
10. Limitation of Liability
CogniHealth is not liable for injuries caused by client environments, third-party equipment, or conditions beyond our control.
To the fullest extent allowed by law, CogniHealth is not responsible for indirect or consequential damages.
11. Termination of Services
Either party may terminate services according to the service agreement.
CogniHealth may terminate immediately due to:
Unsafe conditions
Fraud
Non-payment
Illegal activity
12. Changes to Terms
Updates may be made at any time and posted on our website.
Continued use of Services indicates acceptance of the updated Terms.
13. Governing Law
These Terms are governed by California law.
