Is Safe Transition Home appropriate for this discharge?
Safe Transition Home is a non-medical caregiver-supported service. The patient must be medically cleared for discharge and appropriate for transportation in a standard passenger vehicle.
Typically appropriate when the patient:
- Has been medically cleared for hospital discharge.
- Can safely travel without continuous clinical monitoring.
- Can enter and exit a standard passenger vehicle independently or with reasonable one-person assistance.
- May use a cane, walker or foldable wheelchair but can safely transfer into the vehicle.
- Needs assistance with belongings, entering the residence or getting safely situated at home.
- Would benefit from an essential pharmacy, grocery or supply stop.
Specialized transportation may be needed for:
- Stretcher or gurney transportation.
- A patient who must remain in a non-transferable wheelchair during transportation.
- Continuous skilled medical or clinical monitoring during transit.
- Two-person or mechanical-lift transfers.
